Pediatric ocular rosacea

Purpose: To present a clinical case of a pediatric patient with corneal perforation due to complications of ocular rosacea.
Clinical case: A 9-year-old female patient with a history of brown spots and discomfort in the right eye after 1 week of evolution. Best corrected visual acuity (VA) of the right eye (RE) for hand movements and the left eye (LE) 20/20. Examination with a slit lamp in the RE revealed intense blepharitis, significant telangiectasias on the eyelid margin, watery secretion, moderate mixed congestion, superior pannus, significant neovascularization in the lower half of the cornea with marked thinning of the cornea, inferior paracentral infiltrate area with 4 mm melting, 3 mm diameter perforation with iris prolapse, anterior chamber in atalamia, without evidence of further details. LE mild blepharitis, significant telangiectasias on the eyelid margin, watery secretion, slightly hyperemic bulbar conjunctiva; transparent cornea; formed anterior chamber, intact iris and round pupil, transparent lens. Physical examination revealed erythema on both the cheeks. Therefore, a diagnosis of ocular rosacea complicated by corneal perforation of the RE was made Conclusion: Ocular rosacea is often overlooked by ophthalmologists, especially in children, skin manifestations are not yet evident. Early diagnosis and treatment are important to reduce the morbidity of this disease, which could have serious consequences, such as blindness.
