Objectives: A clinical case report of ocular hypertension following blunt ocular trauma in a pediatric patient who was also detected late congenital glaucoma. To understand the significance of gonioscopic findings in the management of traumatic glaucoma. Clinical case: 10-year-old patient presenting with blunt ocular trauma in the left eye after 72 hours of occurrence: examination revealed ocular hypertension in both eyes (left eye: 24 mmHg; right eye: 35 mmHg). Gonioscopy evidenced multiple goniodysgeneses in both eyes and angle recession in the left eye. Diagnosis of late congenital glaucoma associated with traumatic angle recession in the left eye was made on the basis of gonioscopic findings and clinical examination. The patient was started on topical therapy in both eyes, with good intraocular pressure (IOP) results. Conclusions: The importance of clinical evaluation of any patient who has suffered ocular trauma is hereby stressed, with a special focus on adequate examination of the anterior segment and gonioscopy, since this is how possible complications will be correctly diagnosed and timely treatment will be initiated to prevent visual acuity loss following intraocular pressure elevation.
