Abstract

Objective: To present a case of maculopathy associated with the use of oxytocin in the context of a spontaneous abortion.

Clinical case: A 35-year-old woman presented with a sharp decrease in visual acuity of her right eye with no history of general or ocular diseases, but who 48 hours earlier had been on intravenous oxytocin for 3 hours to induce labor due to a miscarriage at 5 months of gestation. Her ophthalmologic examination revealed extensive premacular hemorrhage, with the rest of the retina intact and no particularities in her other eye. The patient was treated with topical bromfenac 0.09% for 4 months, without visual changes, since she preferred to discard a surgical treatment option. Six years after the event, she remains stable.

Conclusion: Intravenous oxytocin administered during labor may be a trigger for severe macular involvement in the context of a Valsalva maneuver. Excess oxytocin levels at intraretinal tissue may be another pathophysiological mechanism to be studied.
