Abstract

Objective: To present a case of cat scratch disease expressed through Parinaud oculoglandular syndrome.

Case report: A 15-year-old patient consulted for left red eye, febrile condition with parotiditis and ipsilateral adenopathy of 5 days of evolution. Unilateral granulomatous conjunctivitis was observed, representing the expression of Parinaud’s syndrome. Anamnesis revealed a history of contact with a cat. Diagnosis of cat scratch disease was confirmed by serology (IgG of Bartonella henselae 1/256; IgM negative). Treatment with azithromycin 500 mg every 24 hs orally for 5 days was indicated, with resolution of the condition in 2 weeks.

Conclusion: Cat scratch disease should be suspected in cases of unilateral granulomatous conjunctivitis, especially in the context of lymphadenopathy, as seen in Parinaud’s syndrome. Treatment with oral antibiotics was adequate and effective.
