Prevalence of cysts detected in the sulcus of myopic patients

Objective: To evaluate the prevalence and characteristics of cysts detected in the iridociliary sulcus of patients that were under preoperative evaluation for implanting phakic intraocular lenses.
Methods: A retrospective case series study was performed, including myopic patients that underwent preoperative evaluation for posterior chamber phakic intraocular lens implantation between January to October of 2021 in Clinica de Ojos Dr. Nano, Olivos, Argentina. The sulcus anatomy was evaluated by ultrasound biomicroscopic. Any cysts detected were recorded and according to their quadrant distribution were classified as single quadrant cysts (one quadrant) or multi-quadrant cysts (defined as cysts in more than one quadrant). That information was analyzed and in each patient, the unilateral or bilateral condition was considered. Demographic aspects were also described (age, sex, spherical equivalent).
Results: A total of 86 eyes of 43 patients, aged 32.0 ± 7.9 years (21-54) were evaluated. The mean spherical equivalent value was -10.44 ± 4.8 D (-2.5 to -27.5).
In at least one eye, cyst were detected in 24 patients (55.8%); 11 women and 13 men. The presence of cysts were detected in 41 eyes (47.6%) of the total (86 eyes); in 34 eyes cysts were bilateral (82.9%) and 7 eyes had unilateral cysts (17.1%). Regarding their distribution, 17 eyes (41.5%) had single quadrant cysts and 24 eyes (58.5%) had multi-quadrant cysts (2 quadrants in 21 eyes and 3 quadrants in 3 eyes).
In women, 19 cysts were detected: 8 patients bilaterally, 2 cases only in the right eye and 1 in the left eye. In the men population, 22 cysts were found: 9 patients bilaterally, 4 cases only in the OS and none unilaterally in the right one.
Conclusion: Cysts were detected in the sulcus in a high prevalence of myopic patients that underwent preoperative examination for phakic intraocular lenses implantation. Most eyes with cysts had a multi-quadrant alteration. Their postoperative clinical relevance must be studied.
