Postural variation effect on anterior chamber depth and intraocular pressure in patients with pseudoexfoliative glaucoma vs. primary open- angle glaucoma

Abstract

Objective: To determine the differences between eyes with pseudoexfoliative glaucoma (PEG) and others with primary open angle glaucoma (POAG), in sitting vs. supine position.
Methods: A single-center prospective comparative study was performed from April 2021 to May 2022. Patients with GPX (group 1) and GPAA (group 2) were selected.
Variables were compared between both, in different corportal position: anterior chamber depth (ACD) and intraocular pressure (IOP).
Results: Thirty-four eyes of 17 patients were included. The average ACD value in the sitting position was 2.76 ± 0.36 mm (group 1) and 2.32 ± 0.15 mm (group 2). The average ACD increased from 2.76 ± 0.36 mm to 3.04 ± 0.34 mm in GPX eyes and from 2.32 ± 0.15 mm to 2.39 ± 0.17 mm in GPAA eyes with postural change from sitting to supine position. ACD increased in both groups of eyes in the supine position, being more significant in group 1 (p <0.0001 in group 1, p = 0.001 in group 2). IOP in the sitting position was 17.05 ± 2.58 mmHg (group 1) and 14.78 ± 3.53 mmHg (group 2).
The difference in IOP between the sitting and supine positions increased statistically significantly in group 1 but not in group 2 (p=0.0019, p=0.62, respectively).
Conclusions: There was a greater increase in IOP in GPX patients from sitting to supine position than in GPAA patients and a significant increase in anterior chamber depth in the pseudoexfoliation group.
