Abstract

Objective: To present a clinical case of acute hydrops in a keratoconus pediatric patient and describe its management.

Clinical case: 13 year old female patient consulted for decreased visual acuity. Ophthalmological examination determines a decrease in visual acuity in both eyes and confirms corneal thinning, Vogt’s striae and Fleischer’s ring in both eyes. Associated with topographical and pachymetry data, the case was interpreted as keratoconus and it was decided to perform crosslinking treatment in right eye. After 72 hours she presented hydrops in left eye and one month later, a similar case in right eye. Actually is in waiting list for a corneal transplant because developed central leucomas.

Conclusion: Crosslinking is an approved treatment for keratoconus but its efficacy may be different when is used in pediatric or puberal population, as happened in the present case.
