Orbital recurrence of a previously treated retroperitoneal diffuse large B cell lymphoma

Abstract

Objective: To present a case of a patient with a local recurrence of diffuse large B-cell lymphoma (DLBCL), in the left eye, without previous involvement.

Case report: A 79-year-old male, with a history of diagnosis and treatment of retroperitoneal DLBCL, without oculo-orbital involvement, 2 years earlier, consulted for an “orange-salmon” lesion, which was not adherent to deep planes, 10 mm in size in the temporal bulbar conjunctiva of the left eye, painless, with several months of evolution. Vision was preserved (20/20). After performing an incisional biopsy, a diagnosis of germinal-type DLBCL was confirmed. Magnetic resonance imaging revealed involvement of the orbital vertex and lateral rectus muscle. A PET-scan revealed no systemic involvement confirming a diagnosis of local recurrence in the orbital area, previously unaffected. After one cycle of radiotherapy, the patient has achieved a complete response without recurrence after 12 months of follow-up.

Conclusion: In a patient with a history of a previously treated DLBCL, ocular and orbital involvement may be part of the manifestation of lymphoma recurrence in a previously unaffected area. After its ophthalmological diagnosis, this pathology can be effectively treated by the oncology de-partment, as has been observed in the present case.
